Registration Form: REGISTRATION CLOSED DEADLINE MARCH 1ST FOR SPRING, AUGUST 1ST FOR FALL.  NO LATE SIGUPS ACCEPTED. Form for all MCYSL Recreational Soccer Sign ups for Spring or Fall. Please send a copy of childs birth certificate if this is child's first year playing for MCYSL.

Coaches Disclosure:  All new coaches are required to submit a discosure form.  Already approved coaches will need to submit a new form every 2 years. The information should be mailed in an envelope labled:

Players Emergency Information Form: Coaches please notice that on the registration form there is no Medical Information. All players need to fill out a Medical/Emergency form for coaches to keep on them at all your practices and games for Player's Emergency Info. This is a helpful tool, when parents drop their child at a game or practice and go shopping, you (the coach) have contact
information for the child on hand if needed."

Players Medical Release Form: This Form is needed if you Play in any Tournaments.
